ASSUMPTION OF RISK & RELEASE OF LIABILITY
I, the undersigned participant, acknowledge and agree to the following terms and conditions as a member of Flow Roll Studio Jiu-Jitsu, located in Winter Garden, Florida.
1. ASSUMPTION OF RISK
I understand that Brazilian Jiu-Jitsu (BJJ) and martial arts training are physically demanding activities that involve inherent risks, including but not limited to: bodily injury, strains, sprains, fractures, concussions, joint injuries, and in extreme cases, permanent disability or death. I voluntarily accept these risks.
2. RELEASE OF LIABILITY
I hereby release, waive, discharge, and hold harmless Flow Roll Studio, its owners, instructors, employees, agents, and other participants from any and all claims, demands, losses, or liability arising from my participation in training activities, whether caused by negligence or otherwise.
3. MEDICAL FITNESS
I confirm that I am physically and mentally fit to participate in martial arts training. I have not been advised otherwise by a qualified medical professional. I agree to inform the instructor of any health conditions, injuries, or limitations before training.
4. RULES & CONDUCT
I agree to follow all safety guidelines, rules, and instructions provided by Flow Roll Studio instructors. I will train with control and respect for other participants at all times. I understand that unsafe behavior may result in removal from class.
5. PERSONAL PROPERTY
I acknowledge that Flow Roll Studio is not responsible for lost, stolen, or damaged personal property.
6. MEDIA RELEASE
I grant Flow Roll Studio permission to use photographs or videos taken during training for promotional purposes, unless I notify the studio in writing otherwise.
7. MINORS
If I am enrolling a minor, I confirm that I am the parent or legal guardian and accept these terms on their behalf.
8. GOVERNING LAW
This agreement shall be governed by the laws of the State of Florida. If any provision is found unenforceable, the remaining provisions shall remain in full effect.
By signing below, I confirm that I have read, understood, and voluntarily agree to all terms of this waiver. I am 18 years of age or older, or have obtained parental/guardian consent.
